A cidade de Seattle vem sendo aterrorizada por cruéis assassinatos promovidos por Garrote (Jean-Claude Van Damme), que mata jovens mães e depois as incendeia em frente aos seus filhos. Ele é sempre perseguido pelo policial Jake (Michael Rooker) que, apesar de todo seu empenho, não consegue capturá-lo. Mas a chance de enfim prender Garrote chega a Jake quando ele é convidado a participar de um projeto secreto, em que um replicante chamado Número Um (Jean-Claude Van Damme) é criado tendo por base o DNA do próprio Garrote. Número Um, apesar de ter o corpo de um adulto, possui a mente de uma criança que precisa ser educada por Jake para ajudar a enfim capturar Garrote. Porém, como possui os mesmos genes que os seus, Garrote tenta atrair Número Um para aquele que promete ser o seu maior feito.
