Um perigoso psicopata foge de um sanatório e ninguém mais tem notícias dele. Doze anos depois ele reaparece numa cidadezinha do interior espalhando o medo, atacando cruelmente e amedrontando o povo daquele lugarejo com seus assassinatos cruéis. Dois agentes da lei investigam o caso, chefiados pelo sargento Manfredi (Luiz Cargolyver). Mas o maníaco, Douglas, interpretado por Expedycto Lyma, é muito esperto e cuidadoso na sua perseguição doentia por irmãs gêmeas. Até que um dia dá tudo errado e ele senta na graxa. A farra acaba.
