Esta adaptação do romance de Victor Hugo traz a história de dois órfãos, Gwynplaine (Marc-André Grondin), um garoto cuja cicatriz no rosto dá a impressão de que ele está sempre sorrindo, e Déa (Christa Theret), uma garota cega. Em pleno inverno, eles são acolhidos pelo grande Ursus (Gérard Depardieu) e passam a viver com ele. Para ganharem dinheiro, os dois jovens decidem fazer um espetáculo pelas estradas, onde o sorriso de Gwynplaine desperta a curiosidade de todos que passam. Aos poucos, o garoto adquire fama e dinheiro, distanciando-o das únicas duas pessoas que sempre gostaram dele: Déa e Ursus.
