Thomas (Daniel Brühl) é um cineasta em busca de uma segunda chance em Hollywood depois do fracasso de bilheterias de seu primeiro longa. Ele vai a Siena, na Itália, para pesquisar sobre um brutal assassinato que está atualmente em julgamento na cidade. As ordens vêm de seus produtores, em busca de um produto palatável pra vender; a atmosférica cidade medieval e seus habitantes, porém, começam a exercer uma estranha influência sobre o diretor. Ele passa a ter visões e pesadelos desconcertantes enquanto segue em sua busca pela verdade, ao lado de Simone (Kate Beckinsale) uma jornalista que escreveu um livro sobre o assassinato e conhece tudo sobre o caso.
