Após os acontecimentos do primeiro filme, o estranho e introvertido Duane e seu disforme irmão siamês Belial são recolhidos por uma velha médica conhecida como Vovó Ruth, que os leva para uma casa que ela mantém para "indivíduos especiais". Quando um tabloide sensacionalista oferece 1 milhão de dólares a quem localizar os "Gêmeos da Times Square", a repórter Marcie Elliott segue a pista de Duane e Belial até chegar à clínica de Vovó Ruth e ameaça denunciar a bizarra comunidade. Dispostos a defender sua privacidade, Duane e Belial juntam-se aos tipos mais estranhos e arquitetam um plano de vingança.
