Fanatismo Macabro é estrelado pela lendária Tallulah Bankhead (Um Barco e Nove Destinos, série de TV Batman) em sua última performance no cinema. Ela interpreta a psicótica sra. Trefoile, uma mãe louca que aterroriza e aprisiona a noiva de seu falecido filho Pat (Stefanie Powers, da série de TV Casal 20; A Última Diligência) para se vingar da morte trágica do seu filho, com a ajuda de seu jardineiro (Donald Sutherland de Os Invasores de Corpos, Klute – O Passado Condena, Um Inverno de Sangue em Veneza). Uma tirânica fanática religiosa, a sra. Trefoile fica obcecada pelo espírito do filho que morreu há vários anos em um acidente de carro. Quando a ex-namorada de seu filho faz uma inesperada visita, a sra. Trefoile sequestra a bela jovem, confinando-a no sótão como refém para “limpar” sua alma para que ela possa se reunir com seu filho no céu. Presa e torturada, Pat precisa lutar por sua vida e fugir.
