Zatoichi é atacado na estrada por dois bandidos, que ele elimina. Em seguida, ele viaja para uma aldeia cujo médico lhe oferece massagear seus pacientes na clínica. Mas Zatoichi é repetidamente atacado pelo irmão de uma de suas duas vítimas. O “vingador” faz parte de uma gangue de criminosos fugitivos que se refugiaram com o padrinho da yakuza, Matsugoro. Matsugoro concorda em protegê-los com a condição de que eles o ajudem a se livrar de rivais incômodos. Inicialmente um mero observador, Zatoichi gradualmente se envolve nos assuntos de Matsugoro, primeiro forçando-o a libertar uma jovem que ele está explorando à força, depois se opondo aos fugitivos, cujo líder, Shizu, não é outro senão o filho do médico.
