Após enfrentar os monstros sadomasoquistas do inferno que mataram sua família, a jovem Kristy (Ashley Laurence) está em um hospital psiquiátrico, onde não acreditam em sua história. Mas mal sabe ela que o diretor da clínica, Dr. Channard (Kenneth Cranham) é um profundo estudioso do apavorante mundo que Kristy conheceu. Disposto a conhecer tudo por trás dessa história, ele traz de volta a terrível madrasta de Kristy, Julia (Clare Higgins), que passa a matar inocentes. Agora, a jovem se vê novamente correndo risco de passar a eternidade no pior de seus pesadelos.
